      Be Prepared

      To ensure a smooth first visit, please bring the following items with you:

          • Medical insurance card, Medicaid coupon and/or card

          • Proof of income (pay stubs, tax return)

          • Photo ID

          • A list of all your medications

          Payments and Insurance

          We accept various forms of insurance, including Medicare, Tricare, certain private insurance plans, and Medicaid.

              • Georgia Medicaid

              • Humana

              • Peachcare

              • Peach State

              • United Healthcare

              • Wellcare

              If you don’t have insurance, we offer assistance in determining your eligibility for state programs or the Affordable Care Act. Additionally, we provide discount sliding fees based on family size and income level. The Sliding Fee Scale Application is included in the New Patient Registration Forms. Please bring this completed form along with proof of income to your first appointment.
